What is the aim of the program?

How does it work?
This program is designed to enhance the care of kidney transplant patients by promoting self-management. Patients can submit their vital measurements, such as blood pressure, weight, temperature, and urine output, which are then displayed graphically over time for both healthcare providers and patients to review. Patients will receive personalized advice based on their measurements, and our program also includes questionnaires about adherence, smoking, and exercise. In addition, patients can access valuable information about potential complications and travel guidelines. Our aim is to optimize patient care and empower patients to take control of their health.
