Less outpatient clinic visits and care is provided by the right health care professional at the right time

How does it work?
Patients who have tested positive for Covid-19 can be hospitalized (in the ICU). After discharge, a patient remains under follow-up at the hospital for a year. Complaints that a patient may experience after a period of illness can be of both a physical and mental nature. These "extra" patients put a lot of pressure on the outpatient clinics such as pulmonology, internal medicine and medical psychology. Because the patient regularly sends in measurements about how things are going via the Luscii app, outpatient visits are prevented and care can be provided at the right time by the right care provider. Patients receive automated recovery advice via the app in order to increase patient empowerment during their recovery period. When needed the right department is involved in providing the necessary care.
What is the aim of the program?
- Less outpatient clinic visits so more time available for patients who really need care at that moment in time
- Care provided by the right health care professional because of internal triage system
- Improve patient experience because they feel safer upon discharge and have a better recovery experience

Pleasant experience. Seeing whether you went up or down was what I experienced as a kind of check up.
