Push notifications provide information in small, easily digestible bits at moments when patients are paying attention. This may include a video about the day of surgery, a short checklist for consultation preparation, or e-learning about medication if a home measurement is elevated. By playing an active role in their care process, patients can make decisions when possible and take charge of their own health. That is true patient empowerment!

Surgeon Patijn coaches patients around surgery
WHAT'S THE SITUATION?
Dr. Gijs Patijn (Isala) developed a Remote Monitoring program to remotely guide the patient journey around colon surgery. This patient journey includes both the preparatory phase (from diagnosis, including prehabilitation, to admission) and the clinical phase (surgery/ERAS) and finally the recovery phase (after discharge back home).
Prehabilitation helps patients go into major surgery in the best possible condition. More and more research shows that this allows patients to have fewer complications, go home sooner, and recover functionally faster. This is also future-proof for healthcare because it can save capacity and costs.
HOW DOES ISALA DEPLOY TIMELINE?
Prehabilitation consists of physical training, (improving and supplementing) nutrition, mental counseling and possibly quitting smoking and alcohol. The Fit4Surgery foundation (and partners) supports hospitals to further scale up prehabilitation for colorectal and other major surgery. It has developed a special program for this purpose on the basis of which this home monitoring program is designed.
The relatively short-term process surrounding surgery is very intensive and complex for patients. They receive a lot of information. They can be guided optimally when information (often in animations) is offered digitally at the right moments, concisely and in understandable language.
The program is now being expanded to include patient journeys around vascular, HPB, urological, gynecological and cardiothoracic surgeries, including prehabilitation.
